How to Create a Questionnaire In Microsoft Word
Now that you've got your questionnaire template, it's time to customize it to fit your brand. In this next section, we'll go over how you can customize it step by step.
For this tutorial, we'll be using the premium template, Customer Questionnaire.
This premium template is a two page custom document that uses free fonts and is A4 letter size. Let's get started:
1. Edit the Sections
The first step is to add in your questionnaire content to the various sections of the template. The template will have placeholder text that you need to replace. To replace this text, double click the text and then paste in your text.
2. Delete Extra Cells
Not every cell in the questionnaire template will be needed in our document. This will leave blank cells in the tables that you'll want to delete. To delete the empty cells in the templates, right click on an empty cell. Then choose the Delete Cells... option.
3. Customize Fonts
Not every font in the template will be the font that you want to use. To change the font of particular text, first highlight the text. Next, head over to the top main menu header on the Home tab. Click the drop down font menu and choose the font that you want to change the text to.
4. Change the Color Scheme
Now we'll change the color scheme to match our brand's colors. To change the color scheme of the template, click on the Design tab on the top menu header. Next, choose the Colors option. From there a drop down menu appears with a bunch of different color schemes that you can choose from.
5. Add Your Logo
Next, we can add in our logo to brand the questionnaire. Our template has a placeholder for our company's logo. To add in our logo, go to the Insert tab on the top menu. Click on Pictures and choose the Picture from File... option. From there, choose the logo to insert from your hard drive.

Once you find the perfect questionnaire design template, it is time to edit it. Here are some tips to help you get more from your questionnaire:
1. Decide on Audience and Purpose
With any questionnaire, it's essential to decide on your audience and purpose. Or, to put it another way, who do you need information from, and what information do you need. It's worth thinking about this in advance. This is to ensure that you create your questionnaire the right way.
2. Choose Question Types
Once you know what information you need, it's time to create the questions that'll get it. Some questions will require demographic information. Others may provide checkboxes with options to choose from, and others will be open-ended. You'll likely need a mix of questions for the perfect survey.
3. Keep it Short (if Possible)
The shorter your questionnaire is, the more likely it is that people will complete it. So, keep it short if you can. But that's not always possible. If a question HAS to be there to make the survey useful, don't leave it out just to keep it short.
4. Group Similar Questions Together BUT...
If you've got some related questions, group them together in the same section of the questionnaire. Keep the marketing questions together, and the financial questions together, and so on. Mixing them up can cause confusion. This makes it less likely that people will want to complete your questionnaire.
5. Ask One Question at a Time
Multiple questions shouldn't be on the same line. For best results, each question should stand alone. That way it's absolutely clear what people are responding to.
